The current stage of development of psychopharmacological
Professor SN Mosolov Institute of Psychiatry, Ministry of Health, Moscow second half of XX century in the history of psychiatry, without much exaggeration, one can assume that psychopharmacological era. The discovery, intensive study and wider application in the practice of psychotropic drugs has been a truly revolutionary importance for clinical and biological psychiatry. Despite the revealed with the passage of time certain limits its effectiveness, side effects and some ethical doubts about the application, as well as the lack of synthesis of new significantly more efficient means psychopharmacotherapy is currently the most common method of treatment in the world of mental illness. Unlike other treatments for the mentally ill, including psychotherapy and non-drug methods, it is truly massive and casual nature. According to WHO, about 1 / 3 of the adult population in developed countries take psychopharmacological drugs. This is due not only to their relatively high efficiency at a moderate rate of side effects, but with the simplicity and flexibility. It is thanks to the successes of psychopharmacological treatment of mental illness, including severe and chronic forms, it was possible in the community, and the content of patients in psychiatric hospitals was to approach the medical standards. Shifting the center of gravity of the therapeutic process in general practice has allowed more successfully combined with pharmacotherapy and psychotherapeutic methods sotsioreabilitatsionnymi impact, thereby ensuring the unity of biological and psychosocial approaches to treatment and not violating customary for the patient interpersonal relations. Later psychotropic drugs confidently moved out of psychiatric clinics and dispensaries and are actively used by doctors somatic network. Special studies have found that in their application needs about a third of patients with the district policlinics (Smulevich AB et al., 1985, etc.). Another result of the long-term use of psychotropic drugs was changed clinical picture and the most current mental disorders (drug pathomorphosis). Most of nosological forms was leaking at retsudirovannom level and dominated by a relatively favorable course, much more steel to meet light weight, as well as atypical, hidden or masked versions. We can distinguish some basic features of the present stage of development of pharmacotherapy. Firstly, it is a new generation of psychotropic drugs that have qualitatively higher tolerability. And the most significant changes occurred in the area of ??antipsychotic drugs, called atypical antipsychotics, which are clinically effective doses, virtually no causing extrapyramidal side effects, and in respect of antidepressants that selectively block the reuptake of serotonin or noradrenaline and non-holinoliticheskimi, adrenolytic and other side effects of drugs tricyclic structure.
